Dear Doctor,

In my reports for my patients, I like to do a “final thoughts” as a final summary to the patient to indicate the re-test time period and any dietary additions or temporary supplement suggestions. I’ve provided these templates in this folder that will make this task easier for you should you choose to do this for your patients.

There are 3 templates. Template 1 should be used when you test only the Blood and Hair samples. Template 2 should be used if you test Blood, Hair and Urine Toxic Challenges. Template 3 is used when you only do a Urine Toxic Challenge.

Read through each template carefully and delete impertinent data and fill in the blanks as needed. In other words, “fine tune” the document to fit that patient’s unique situation. When finished, remember to “Save As” and rename the saved document so as not to lose the original template. Save the Final Thoughts in the patient’s electronic file.

The final thoughts should be inserted in the report where the vitamin list prints out, the page before the colored blood lab chart.

We truly want you excel in your nutrition practice. Please let us know how we are doing and what we can do to help.

Yours in health,

Dr. Van D. Merkle
